Recommended Links List - options

I refer to the recommended link list options section appearing at the top of search results appearing in response to specified search words (phrases).

For the moment it appears one can nominate certain titles, links and attach descriptions thereon together with a specified number of items that appear.

I would like a form to appear instead of a link(s). See here the 'need a car loan' form:

http://www.usedautodirectory.com/

where users can submit their details in different boxes.

Is this possible?

If it is always the same form, (and doesn't vary be search word) then you can just insert the form into the search template file.

No, we would like a form to be called per search term. Thanks for your response. We understand it's not easy but worth asking the question.
