Confused - No search form on search page

I'm trying to use this search application for my web-site, I know I'm doing soemthing wrong but I can't see where I've taken the wrong turn.
I've uploaded all the indexes and .asp file, the .html file and when I go to preview it on the site all I get is:

Search this site
Enter one or more keywords to search for using the Zoom Search Engine.
Note that '*' and '?' wildcards are supported.

Please help, I don't know what I'm doing!
