Custom Search Form - Two search forms

I've created a custom search form:
http://www.cjrfinearts.com/search.htm

However, when a search is done, the results page displays 2 forms? How do I get rid of this?

The results page should be looking to:
http://www.cjrfinearts.com/search_template.html

Nevermind, I figured it out. My search_template.html also had the <Form> tags.
